of Mailing Affidavit (from the company who processed and served
your Preliminary Notice). |

Copy
of Contract or Scope of Work (if no Contract). |
| 5. |
Copy
of all invoices that calculate to the total Lien amount. |

of change orders that calculate to Lien amount (if applicable). |
